Heart Health Checks

Heart disease is a major health concern in Australia, contributing to one in four deaths and leading to 1,619 hospitalisations daily. With over half of Australian adults living with three or more modifiable risk factors for cardiovascular disease (CVD), it’s clear that proactive management is crucial. Modifiable risk factors, such as high blood pressure, smoking, and poor diet, account for up to 90% of the risk of heart attack, making CVD largely preventable with appropriate interventions.

Despite this, many Australians are not fully aware of the risks—only 5% recognise high blood pressure as a key risk factor, and 31% identify smoking as a significant concern. Our Heart Health Checks are designed to address this gap in awareness and prevention.

Eligibility for Heart Health Checks:

  • Adults aged 45 and over who are not already known to have CVD.
  • Aboriginal and/or Torres Strait Islander Peoples aged 30 and over.

Each appointment lasts at least 20 minutes and includes:

  • Collection of relevant information, including a detailed patient history,
  • A basic physical examination, which includes recording blood pressure and cholesterol levels,
  • Initiation of appropriate interventions and referrals based on the assessment,
  • Development and implementation of a personalised management plan; and
  • Provision of preventative health care advice and information to help manage and reduce risk factors.
